ABOUT THE ROLE:

Exciting opportunity for a Senior Social Worker to join our newly established Hospital Outreach Post-suicidal Engagement (HOPE) Team based at Werribee Mercy Hospital. The role of the Senior Social Worker is to provide mental health and psychosocial assessments, evidence based interventions and facilitate linkages in the community to support recovery goals.

  • Part time role (0.8 EFT), fixed term for 2 years (commencing February 2019)

  • Hospital Outreach Post-suicidal Engagement (HOPE) Team

  • Based at Clare Moore Building at Werribee Mercy Hospital

  • Work in collaboration with other senior clinicians and support workers to provide assertive, tailored postvention support in the community.

  • Provide evidence based intervention sessions such as crisis and systemic interventions, counselling strengths based interventions, care coordination, facilitate community engagement, social inclusion whilst promoting resilience.

ABOUT YOU:

  • Approved degree in Social Work completed and evidence of eligibility for membership of the Australian Association of Social Workers.

  • Experience working in community mental health services.

  • Post graduate qualifications or working towards same.

  • Other relevant tertiary qualifications or working towards, in relevant areas such as therapeutic interventions, drug and alcohol or psychosocial interventions.

  • Current unrestricted Victorian driver’s license.
