As part of the Health Independece Program (HIP) team you will provide and support the delivery of best practice in centre and home based programs to HIP clients within the Wyndham catchment. You will be based in a dynamic office working alongside a multidisciplinary team of Allied Health staff and Specialist Nurses.

This is a high performing team working Monday – Friday office hours, weekends and public holidays off. HIP consists of 5 services, which are Community Based Rehab, Complex Care Coordination, Specialist clinics: Falls and Balance and Continence and Residential InReach. The role main role is to provide Physiotherapy to the clients of the Complex Care service.

• Short term contract 0.4EFT until 7 September 2018 ( maternity leave)
• Deliver centre and home based physiotherapy services to a diverse client group with complex health needs.
• Improve self –management skills of clients with chronic diseases
• Expertise in delivering pulmonary rehabilitation preferable
• Use of motivational interviewing techniques to help clients explore and resolve their uncertainty

ABOUT YOU:
• Recognised Degree in Physiotherapy, current, full physiotherapy registration with AHPRA
• Current Victorian (or equivalent) Drivers Licence
• Strong, relevant clinical skills and experience in sub-acute/ community setting
• Highly developed communication and interpersonal skills,
• Experience and knowledge in the management of people with complex chronic care needs
• Commitment to person-centred care and ability to adapt and learn quickly with an ability to work autonomously and flexibly within an inter-professional team
